We want you to know how you can get help and what to expect during your visits with us.

What to Expect

The first person you will meet when coming to our office is a member of our support staff. They will help you complete your registration paperwork and get you ready to see your provider.

If you are coming to Allwell for behavioral health or substance use services, the first time you come in, you’ll see an outpatient counselor who will listen to your story and will work with you to develop a treatment plan.

If you are coming to Alwell for your physical health, your first visit will be with your primary care provider. This provider will do a health assessment with you and do a thorough examination to determine your healthcare needs and treatment plan.

What to Bring With You

Make sure you bring the following with you to your appointment:

  • Insurance cards
  • Photo ID
  • Social Security Number
  • Proof of Income
  • Proof of Guardianship and/or custody papers
  • Relevant court documents (i.e. Court Orders)
